OHP Referral Form for èßäÊÓƵ Staff

  • PPS now has a direct referral system for PPS staff to submit referrals to OHP Assisters for families who need support:

    • Getting Enrolled in OHP or Marketplace
    • Systems Navigation to access Health Services

    Important things to know

    OHA/CPOP cannot accept referrals without prior consent to release Referred Individual Information by PPS. 
    OHP Referral Form: Cannot move forward with the referral if answer is “No” to this question.
